  • Incorrect or incomplete patient information: A wrong DOB, misspelled name, or incorrect insurance ID can stop a claim from processing or cause it to be denied later.
  • Diagnosis code issues: The diagnosis may be clinically correct, but not aligned with payer requirements for the service billed.
  • Missing or incorrect modifiers: A small oversight that can lead to reduced payment or full denial.
  • Payer processing errors: Yes, they happen. Claims can be misprocessed, applied incorrectly, or denied in error.

The hidden cost: Time

Identifying why a claim wasn’t paid is where the real burden begins.

A single claim often requires:

  • Reviewing the Explanation of Benefits (EOB)
  • Cross-checking the original submission
  • Verifying eligibility and benefits
  • Confirming coding and modifier accuracy

Time required: 15–30 minutes per claim

If a call to the insurance company is needed:

  • Hold times
  • Transfers
  • Conflicting information

Add another 20–45 minutes.

What’s the correct next step?

  • Resubmit the original claim?
  • Submit a corrected claim?
  • File a new claim?
